How to enter your team:

1 - Print and complete the following Entry Forms per instructions (eg Indemnity Form (b) - Parent/Guardian to sign)

• Champions Cup Nomination Form for 2008
• or Wallaroo Nomination Form 2008

          Please send your  Nomination Form with $100AUD  deposit

We cannot accept nomination without deposit.

• Team Entry Details form 2008
• Indemnity Forms for players - Parent/Guardian must sign
• Indemnity Forms for Coaches/Managers/Officials

2 - You are required to provide with the forms, the following support information for each player in your team:

• Photocopy of your Federation's Registration list confirming your players registration;
• Or Copy of your School's Enrolment form to confirm the ages of your students.
• Or Proof of Age documents and photographs.

4 - Send the forms with the balance of fees to:

FEES

Gold Coast Champions Cup Entry Fee $800 includes insurance for 14 players and 2 officials any extra players/officials incurs a cost of $15 per person.

Downunder Cup Entry Fee $900 includes insurance for 14 players and 2 officials any extra players/officials incurs a cost of $15 per person.

Whilst the number of players in a team is set by the rules of the competition, for administration purposes we request a minimum payment of Player & Officials Fees based on 14 players and 2 Officials to be paid with the balance of the Entry Fee.

Wallaroo Cup Entry Fee $450 includes 11 players and 2 officials any extra players/officials incurs a cost of $15 per person.  

5 - Be able to provide proof of player age. (Have with you a copy of your Federation or School player registration information).
