Entry Details

1) Complete the Entry Forms relevant to the competition you are entering (below) in one of two ways:

• Complete ONLINE and submit them via email, using the SUBMIT button at the top of each form
• Or PRINT and complete the - plus all other forms and documentation as defined in this checklist and fax/mail it to:

• Champions Cup Nomination Form for 2010
  The Champions Cup is the original format 11-a-side tournament for boys and girls teams U9 through U21.
  
  
• Beach Soccer Nomination Form

Please send your  Nomination Form with $100AUD  deposit - We cannot accept nominations without the deposit and your team will NOT be registered until the deposit has been received.

• Team Details form (Players, Coach/Officials) 
• Indemnity Forms for players - Parent/Guardian must sign
• Indemnity Forms for Coaches/Managers/Officials

2) You are required to provide with the forms, the following support information for each player in your team:

• Photocopy of your Federation's Registration list confirming your players registration;
• Or Copy of your School's Enrolment form to confirm the ages of your students.
• Or Proof of Age documents and photographs.

3) Obtain written permission from your local Federation or School to travel to and participate in the sanctioned tournament.

4) Send the forms with the balance of fees to:

Insurance

Gold Coast Champions Cup provides a level of insurance to all properly registered attendees. However, it is important that each team entering the tournament ensures that they have adequate cover through their FFA or FIFA affiliated insurer. Our insurance details can be found here.

FEES

Gold Coast Champions Cup Entry Fee $985 includes insurance for 14 players and 2 officials any extra players/officials incurs a cost of $20 per person.

Whilst the number of players in a team is set by the rules of the competition, for administration purposes we request a minimum payment of Player & Officials Fees based on 14 players and 2 Officials to be paid with the balance of the Entry Fee.

5) Be able to provide proof of player age. (Have with you a copy of your Federation or School player registration information).
